认知行为团体辅导对留守学生弱视患者的干预效果

摘    要:
目的:探究认知行为团体辅导对于提高留守学生弱视患者治疗积极性及改善患者弱视情况的效果。方法:选取2013年1月至2015年12月于我院接受治疗的86例留守学生弱视患者(104眼)为研究对象,按照随机数表法将患者平均分为两组,每组各43例。两组患者均加以扩瞳检查、验光配镜等常规弱视治疗手段,对照组采用常规护理手段,观察组在此基础上加以团队认知行为疗法进行护理。护理1个月后采用依从性量表、焦虑自评量表(SAS)和抑郁自评量表(SDS)分别对两组患者的治疗依从性、焦虑程度和抑郁程度进行评定;采用自我效能量表评定两组患者包括社交效能、饮食选择、保持活力信心、照顾效能等多项指标;对两组患者弱视恢复情况进行比较。结果:护理1个月后,两组患者在依从性、SAS、SDS得分上有显著差异(t=2.909,-8.14,-6.127;P0.01)。两组在自我效能量表评分上具有统计学差异(t=7.112,5.817,4.187,9.91,3.851,4.193;P0.01)。弱视恢复情况的比较发现,观察组患者基本痊愈22眼(44.0%)、有效21眼(42.0%)、无效7眼(14.0%)、总体恢复率86.0%,对照组患者基本痊愈17眼(31.5%)、有效20眼(37.0%)、无效17眼(31.5%)、总体恢复率68.5%,两组数据比较不存在统计学差异(χ~2=4.685,P=0.096)。结论:认知行为团体辅导能够有效提高留守学生弱视患者的治疗积极性,提高患者自我效能,对于弱视恢复有积极作用。

Abstract:
Objective:To explore the effect of group cognitive behavior therapy in improving the treatment of left behind children with amblyopia and improving the situation of amblyopia.Methods:86 cases of left-behind children with amblyopia from January 2013 to December 2015 in our hospital were selected as the research object,according to the random number table method,the patients were divided into two groups,43 cases in each group.Two groups of patients were both given mydriatic optometry examination,routine treatment of amblyopia.Control group were given conventional care,the observation group were given a team of cognitive behavior therapy for nursing on this basis.A month after the nursing,compliance scale,self rating Anxiety Scale(SAS)and self rating Depression Scale (SDS)were used to detecte the two groups of patients treatment compliance,degree of anxiety and depression,two groups social efficiency,diet selection,maintainning the vitality of confidence and efficacy index were compared;two groups of patients with amblyopia recovery were compared.Results:After a month of nursing,there were significant differences in compliance SAS score and SDS score between two groups(t = 2.909,-8.14,-6.127;P < 0.01 ).There were significant differences in self- efficacy scale scores between two groups(t = 7.112,5.817,4.187,9.91,3.851, 4.193;P <0.01).The patients in the observation group were cured in 22 eyes(44%),effective in 21 eyes(42%),inva-lid 7 eyes(14%),the overall recovery rate of 86%,control group were cured in 17 eyes(31.5%),effective in 20 eyes (37.0%),invalid 17 eyes(31.5%),the overall recovery rate of 68.5%,the data compared between the two groups was not statistically significant(P >0.05).Conclusion:Group cognitive behavioral therapy can effectively improve the treatment of left behind children with amblyopia and improve the self-efficacy of patients,and has a positive effect on the recovery of amblyopia.
