重度出血性膀胱炎的介入治疗

目的 探讨双侧髂内动脉栓塞在重度出血性膀胱炎(HC)治疗中的作用.方法 11例经内科保守治疗无效的HC患者接受了双侧髂内动脉栓塞.其中包括慢性粒细胞白血病4例,急性粒细胞白血病2例,急性淋巴细胞白血病1例,宫颈癌患者1例,前列腺癌患者3例.结果 4例患者肉眼血尿消失；3例患者肉眼血尿减少；1例无效;3例复发.未见明显与栓塞治疗直接相关的并发症.结论 采用双侧髂内动脉栓塞治疗重度出血性膀胱炎,栓塞效果好、不良反应小,是在行手术切除膀胱前可供选择的治疗方法.

Bilateral internal iliac artery embolization in the treatment of severe hemorrhagic cystitis

Objective To assess the value of bilateral internal iliac artery embolization in the treatment of hemorrhagic cystitis. Methods 11 patients with CML （n=4）, ALL （n=2）, AML （n=l）, cervical cancer （n=l）, and prostate cancer（n=3 ）underwent bilateral internal iliac artery embolization for severe hematuria uncontrolled by conservative treatment. Selective embolization of the internal iliac arteries was perforthed using gelatin sponge. Results Four patients recovered completely with cessation of hematuria after embolization. Three patients improved partially, one patient did not respond and hematuria recurred in 3 patients. Conclusion Selective embolization of the internal iliac arteries is a treatment option before surgery in patients with severe hemorrhagic cystitis.
