腹腔镜联合胃镜手术治疗胃间质瘤27例分析

目的：讨论胃镜引导下腹腔镜手术治疗胃间质瘤的可行性。方法回顾性分析2010年3月至2014年3月期间，在电子胃镜导引下行腹腔镜手术治疗的27例胃间质瘤患者的临床资料。结果27例胃间质瘤患者在胃镜引导下利用腹腔镜成功切除，肿瘤切除完整，边缘周围无残留。手术切口长约3～6 cm，手术时间40～110 min，术后患者恢复顺利，未发生任何并发症。术后住院时间4～7 d，平均5.5 d。随访3～36个月，未发生复发或转移。结论单纯腹腔镜下难以定位的胃间质瘤，可以在电子胃镜引导下进行手术，并且可以使切除范围更加精确完整，避免切除过多正常胃组织和肿瘤残留。

Analysis of 27 cases of stromal tumor laparoscopy combined with electronic gastroscope treatment of stomach

Objective To summarize the feasibility of the gastroscope guided laparoscopic surgery in the treatment of gastric stromal tumor. Methods Retrospective analysis of our hospital from 2010 March to 2014 March,clinical data of 27 cases of gastric therapy of stromal tumor patients in gastroscopy guided by laparoscopic surgery. Results Successful resection was obtained in 27 cases of gastric stromal tumour patients with endoscopic laparoscope,total resection without residual around,Surgical incision of about 3~6 cm,40~110 min,operation time,postoperative recovery was smooth,without any complications. The postoperative hospital stay was 4~7 d,average 5.5 d. Patients were followed up for 3~36 months,no recurrence or metastasis. Conclusion Laparoscopic difficult positioning of gastric stromal tumor,surgery can be guided by electronic gastroscopy,which can make more accurate and complete resection range,avoid excessive resection of normal gastric tissue and residual tumor.
