老年消化性溃疡

报告我院1958～1979年间经手术和/或X线钡餐检查、纤维胃镜或尸检证实的消化性溃疡1,263例,着重对老年(≥60岁)与青年住院患者的临床特点进行比较。老年人以胃溃疡多见,症状多不典型,其合并症、急性穿孔发生率和手术死亡率均高于青年人(p<0.001)。

PEPTIC ULCER OF THE AGED

A total number of 1,264 inpatients of peptic ulcer were studied in a period of twenty-one years (1959-1979). The diagnoses of these cases were established by operation and/or barium meal examination. Some were diagnosed by endoscopy or autopsy. Of these 1,263 cases, 92 (7.28%) were over the age of 60. Comparing this aged group with the younger group under age of 30 (197 cases, 15.60%), the incidence of gastric ulcer was higher in the aged group than in the younger group while the incidence of duodenal ulcer in the younger group was higher. The symptoms of peptic ulcer in tie aged group were atypial. The incidence of perforation of peptic ulcer was also higher in the aged group(P<0.01), but there was no significant difference in the incidence of upper gastrointestinal bleeding between the two groups (P> 0.05). The percentage of gastric cancer accompanied with gastric ulcer in the aged group was 2.17%, but none in the younger group. The mortality of complications, of operation and of associated diseases were also apparently higher in the aged group (P<0.001). Therefore, it is important to choose the optimal time of operation.
