局麻胸腔镜术治疗难治性结核性胸膜炎385例分析

目的介绍局麻胸腔镜术治疗难治性结核性胸膜炎的方法和疗效。方法对385例难治性结核性胸膜炎行局麻胸腔镜手术。根据术前治疗情况,分A穿刺组,B引流组,C直接胸腔镜术组。三组行局麻胸腔镜治疗方式相同,对比恢复情况、住院天数、住院费用。结果 385例局麻胸腔镜术均顺利完成,三组的术后主要肺功能指标恢复情况无差异,但C直接胸腔镜术组住院天数、住院费用均明显降低,有显著差异。结论局麻胸腔镜术治疗难治性结核性胸膜炎是安全、有效的方法,可明显降低住院天数和费用。

Analysis of Thoracoscope under Local Anesthesia in treatment of 385 Cases of Refractory Tuberculous Pleurisy

Objective To explore the method and effect of local anesthesia-assisted thoracoscopic on refractory tuberculous pleurisy.Method 385 patients with refractory tuberculous pleurisy undergone local anesthesia-assisted thoracoscopic.All patients were divided into group A(puncture group),group B(drainage group) and group C(direct thoracoscopic surgery group) according to the preoperative treatment condition.postoperative recovery,length of stay and cost of hospitalization were copared among three groups.Results 385 cases of thoracic surgery under local anesthesia were carried out successfully and no differences in postoperative recovery of the major lung function among three groups were observed,but length of stay and cost of hospitalization in group C were the lowest with significant differences compared with group A and group B.Conclusion Treatment of thoracoscopy under local anesthesia on refractory tuberculous pleurisy is a safe and effective method,which can significantly reduce the number of hospital days and costs.
