血清白蛋白、前白蛋白水平在AECOPD的临床意义

目的探讨血清白蛋白(ALB)、血清前白蛋白(PA)水平在COPD患者急性加重期的临床意义。方法入选COPD急性加重期老年男性患者52例,比较其血清ALB、PA浓度与患者住院时间及心衰纠正时间的相关性,并分析ALB、PA与患者动脉血PH值、PaO2、PaCO2之间的关系。结果低ALB、低PA组患者的平均住院时间及心衰纠正时间明显延长,差异有统计学意义(P<0.05)。ALB正常组与ALB降低组血气PaCO2存在显著性差异(P<0.05),PA正常组与PA降低组血气PaO2存在显著性差异(P<0.05)。结论血清ALB、PA水平可作为评估COPD急性加重期患者预后的一项指标,PA明显降低更加提示预后不良。

Study on the clinical significance of serum albumin and prealbumin in AECOPD patients

Objective Study on the clinical significance of serum level of albumin(ALB) and prealbumin(PA) in acute exacerbation of chronic obstructive pulmonary disease(AECOPD) patients.Methods 52 aged male patients of AECOPD were selected and the relationship between hypoproteinemia and the time of hospital stay and heart failure was examined,the relationship between the level of ALB and PA and blood gas analysis(PH,PaO2,PaCO2) was investigated.Results The mean time of hospital stay and heart failure in the cases with low ALB and PA were remarkably longer than those with normal,significant difference(P<0.05) was shown.In the group of low ALB,PaCO2 was significantly higher than the group of normal(P<0.05).In the group of low PA,PaO2 was significantly lower than the group of normal(P<0.05).Conclusions The level of serum ALB and PA can be used as assessment of AECOPD severity and prognosis.Especially PA could play a more important role in the occurrence and development of AECOPD.
