侧脑室—腹腔分流治疗脑积水62例报告

目的　探讨脑积水的治疗方法和疗效。方法　62例均行侧脑室-腹腔分流术。脑室端从枕角穿入10cm接引流泵,再从皮下隧道引至上腹或左下腹放入腹腔。结果　1～4周后复查,症状完全消失56例,部分改善5例,无变化1例。57例复查CT示:52例脑室恢复正常,4例脑室较前缩小,1例无变化。堵管9例。低颅内压2例,硬膜下血肿2例,经保守治疗痊愈。49例随访1～8年,41例能正常工作学习,5例生活自理,1例植物生存,2例死亡。结论　明确诊断,掌握手术适应症及手术技巧是成功的关键。

LATERAL VENTRICLE-PERITONEAL CAVITY SHUNT FOR THE TREATMENT OF HYDOROCEPHALUS-WITH A REPORT OF 62 CASES

Objective To explore the therapeutic method and effectiveness for the treatment of hydrocephalus.Methods 62 cases were treated with lateral ventricle-peritoneal shunt.The drainage-pump was put into the termination of ventricles at 10 cm from the occipital angle and then put into the peritoneal cavity from the subcutaneous tunnel through the epigastrium or the hypogastrium.Results A reexamination of 1-4 weeks after shunt showed that the clinical symptoms disappeared in 56 cases,in 5 cases improved partly and in one case there was no improvement.In 57 cases of reexamination,the CT examination showed that in 52 cases the ventricle of brain returned to normal,in 4 cases shrank more than before and in one case there was no improvement.In 9 cases,obstruction of shunting tube occurred and in 2 cases and in another two cases,intracranial hypotension and subdural hemoto occurred respectively,which were cured with conservative treatment.A follow-up survey of 1-8 years on 49 cases showed that of all the cases,41 cases could live a normal life,5 cases could look after themselves in their daily life,one case lived a vegetal life and two cases died.Conclusion Lateral ventricle-peritoneal shunt was an effective way in the treatment of hydrocephalus.The success in the shunt operation depends on the confirmed diagnosis,operative skill and the mastering of the indications and contradictions of operation.
