胰腺囊性及囊实性病变的超声诊断

目的：探讨超声对胰腺囊性及囊实性病变的诊断价值。材料与方法：超声检查38例胰腺囊性及囊实性病变,并经手术病理证实。记录比较各类病变的不同超声表现。结果：胰腺囊腺瘤及囊腺癌10例,主要表现为境界较为清晰的囊性包快,亦可表现为囊实混合结构,实性部分质地较为疏松,呈＂豆腐渣＂样改变。胰腺癌10例,主要表现为以实质低回声为主的不规则肿块,内回声不均匀,可有小片状液性暗区。胰腺单纯性囊肿1例,表现为边界清楚的囊状暗区,壁光整,暗区清晰,无分隔。胰岛细胞瘤1例,表现为囊实混合性包快,边界清楚,内有较多中低光点及不规则液性暗区并可见分隔光带。胰腺假性囊肿16例,主要表现为大小不一的囊性包块,少数亦可为囊实混合结构,部分包块暗区清晰,部分包块暗区内回声混杂。结论：超声对囊性及囊实性病变有较高诊断价值。

Diagnosis Value of Ultrasound for Pancreatic Cystic and Solid-Cystic Diseases

Purpose：To evaluale the value of ultrasound in the differentiating diagnosis of pancreatic cystic and soliod-cystic lesions.Materials and Methods：38 of pancreatic cystic and solid-cystic lesions confirmed by operation and pathology,the ultrasonographic charactistics of different diseases were recorded and compared.Results：All the 10 cystoademoma and cystocarcinoma were shown as cystic mass or mixed construction,the solid part was loose in the masses.The pancreatic cancer（10 cases）were charactized by solid mass of lower echo mianly,but mixed construction also was shown sometims.The pancreatic cyst（1 case）was shown as cystic area with clear mangin and clear liquid.The islet cell tumors（1 case）was shown as mixed mass with clear margin and many medium-lower echo and irregular liquid area.Almost all the 16 pancreatic pseudocyts were shown as cystic mass,but its also may be mixed construction in the cases of a small number,the liquid in the mass was clear or complex.Conclusion：Ultrasound diagnosis value is very important to pancreatic cystic and solid-cystic diseases.
