| 摘 要： | 目的:探讨胆道镜技术对胆道疾患术式选择的作用。方法:收集术中或联合术后应用胆道镜技术治疗的胆道疾患182 例临床资料,进行分析及总结。结果:134 例胆管胆石症患者胆道残石率从35 .2% 降至9.7% ;术中胆道镜发现30 例合并胆管狭窄,其中15 例肝胆管膜状狭窄者应用在胆道镜指导下实施了有效扩张治疗;13 例术后残留结石取尽12 例;胆总管下段梗阻中24 例肿瘤依据形态学检查,结合术中扪诊或联合病理学检查得到确诊并采取了相应的手术方法。结论:胆道镜技术的应用有利于胆道疾患术式的选择,可使一些较为复杂的胆道手术变得简单易行;对术前未能确诊的病变可以达到明确诊断并指导选择合理的术式 |

| Abstract: | Objective: To evaluate the role of choledochoscope in the planning of surgery and therapy for biliary diseases. Methods: 182 cases treated by choledochoscope during or after operation were analyzed. Results The rate of residual stone decreased significantly from 35.2% to 7.2% by using choledochoscope. The causes of biliary obstruction were diagnosed by choledochoscope and relevant operations were performed in 40 cases. Conclusions: Choledochoscope is an important technique in planning the therapeutic program for biliary diseases, it is helpful for simplifying some complex biliary operations. Further more, some indefinite biliary diseased including bileduct obstruction can be diagnosed and be directed to get correct therapy under choledochoscope. |
