高血压社区管理和综合干预对高血压患者的控制疗效研究

目的：探讨高血压社区管理和综合干预对高血压患者的控制疗效。方法选取2013年2月~2014年2月本社区的257例高血压患者作为研究对象，为其建立档案，详细记录其当前血压和各项生化指标，在1年内给予运动、用药、饮食、健康教育、心理等指导，观察并比较干预前后患者的知晓率、治疗率、控制率、遵医嘱率以及血糖、血脂的好转率等。结果经有效干预后，患者对高血压的知晓率显著增高，其血压由（170±8） mm Hg下降至（135±11）mm Hg，治疗率由30.7%上升至68.5%，控制率则由20.2%上升至40.5%，高血脂以及高血糖也趋向好转，干预前后的各项指标比较差异有统计学意义（P〈0.05）。结论对社区内的高血压患者采取规范化的社区管理和综合干预不仅能够有效控制血压，还能够提高患者的生活质量，值得临床推广应用。

Study on control efficacy of hypertension community management and comprehensive intervention for patients with hypertension

Objective To explore the control efficacy of hypertension community management and comprehensive inter-vention for patients with hypertension. Methods 257 patients with hypertension in the community from February 2013 to February 2014 were selected as research objects.The present blood pressure and biochemical indexes were recorded. The patients received the guidance of movement,medication,diet,healthy education and psychology in one year.The awareness rate,treatment rate,control rate and rate of following the doctor’s advice,and the improvement rate of blood sugar and blood fat of patients before and after intervention was observed and compared. Results After effective inter-ventions,Rate of patient awareness of hypertension significantly increased,blood pressure decreased from（170±8） mm Hg to （135±11） mm Hg,the treatment rate increased from 30.7% to 68.5%,the control rate increased from 20.2% to 40.5%, high cholesterol and high blood sugar also tended to improve,the difference before and after the intervention were sta-tistically significant （P〈0.05）. Conclusion Specified community management comprehensive intervention for patients with hypertension in community not only can effectively control the blood pressure,but also can improve the life quality of patients,which is worthy of clinical promotion and application.
