关节镜下髌骨外侧成形术治疗髌股关节骨关节炎

目的介绍关节镜下髌骨外侧成形术治疗髌股关节骨关节炎的方法，总结其初期临床结果。方法2009年1月至2010年2月，解放军309医院共收治32例59膝膝关节髌股关节骨关节炎患者，女26例，男6例，年龄36-54岁，平均45岁，术前诊断髌股关节骨关节炎，采用关节镜下关节清理，髌骨外侧成形术治疗，术后康复训练。结果25例患者（48膝）获得随访4～16个月，平均8个月，Lysholm评分：术前54．6±4．3，术后87．6±2．8（P〈0．01）。结论关节镜下关节清理，髌骨外侧成形术，可以明显减轻髌股关节骨关节炎引起的疼痛，取得满意的临床效果。

Treatment of patelo-femoral ostarthritis through lateral patelloplasty under an arthroscope

Objective To explore the treatment of patelo-femoral ostarthritis through lateral patelloplasty under an arthroscope and to summarize the early clinical effectiveness. Methods Between Jan 2009 and Feb 2010, 32 cases of pa tello-femoral ostarthritis （59 knees ） were treated by lateral patelloplasty under an arthroseope. There were 6 males （8 knees） and 26 females （32 knees） aged 36 to 54 （45 years old on average）. We started the operation by cleaning the artic ular cavity, performed arthreetomy on the lateral patella about 1 cm upward, followed by mechanical gouging on the disor-der cartilage of patella and femoral trochlea. All the cases received a strictly-staged knee functional rehabilitation. Twenty five cases （48 knees） were followed up for 4 -16 months （mean 8 months） . The results were evaluated according to the Lysholm knee score. Results The mean Lysholin knee score was improved from 54.6 ± 4.3 preoperatively to 87.6±2.8 at the last follow-up （P 〈 0.01 ）. Conclusion Lateral patelloplasty under an arthroscope has the advantages of less injury, quick knee joint function recovery and good effect on patelo-femoral ostarthritis.
