焦作市机采血小板检测结果回顾性分析

目的对无偿献血者机采血小板检测结果进行回顾性分析,探讨机采血小板因检测结果不合格而造成报废的原因及对策。方法对2011年1月～2013年12月机采血小板的检测结果进行统计分析,对重复检测结果不合格的献血者进行跟踪观察。结果 （1）机采血小板献血者4508例,共有30例因检测结果不合格而报废,不合格率为0.67%,其中抗-HIV阳性1例,其余29例均为弱反应性。HBsAg弱反应性8例、抗-HCV弱反应性7例、抗-HIV弱反应性3例、抗TP弱反应性11例;（2）发现7例重复弱反应性机采献血者,其弱反应性项目均与前一次相同,且两次不合格间隔时间较短,多在6个月以内。结论 （1）机采血小板因检测结果不合格导致的报废在酶免四项中均有存在,且多为灰区所致。应根据实验室情况合理设置灰区,同时选用灵敏度高、特异性好的检测试剂,既保证检测结果的准确性,又减少非特异性假阳性反应造成的血液报废;（2）对机采血小板献血者应加强管理,采取多种措施以降低因检测结果不合格导致的报废,节约有限的血液资源及昂贵的试剂耗材。

Retrospective analysis of apheresis platelet detection results in Jiaozuo city

Objective To explore the reasons and countermeasures for scrap of apheresis platelet caused by unqualified test result,Apheresis platelet test results of unpaid donors were retrospectively analyzed. Methods Test results of apheresis platelets from 2011.1 ～ 2013.12 were used for statistical analysis,the blood donors who＇s blood was duplicate detection unqualified were trace observed. Results（1）There are 4508 apheresis platelet donors,a total of 30 cases of blood was discarded because test result is unqualified,Non-qualified rate is 0.67%,1 case was antiHIV positive,the remaining 29 cases were weakly reactive.8 cases were HBsAg weakly reactive,7 were anti-HCV weakly reactive,3 were anti-HIV weakly reactive, 11 were TP weakly reactive.（2）We found 7 cases apheresis platelet donors of repeated weakly reactive,the weak reactive project are the same as the former one,and the times interval of unqualified was short,mostly in 6 months. Conclusion Scrap caused by unqualified test results were in Enzymes from four,and mostly in the grey area.Setting the gray zone reasonable under laboratory conditions,at the same time using high sensitivity,good specificity detection reagent,can both ensure the accuracy of test results,and reduce the blood discarding caused by nonspecificity false positive reaction.（2）We should strengthen the management For apheresis platelets donors,take various measures to reduce the scrap caused by failed testing results,and save the limited blood resources and expensive reagent.
