细胞性良性纤维组织细胞瘤临床病理特征

目的探讨细胞性良性纤维组织细胞瘤(celluar benign fibrous histiocytoma,cBFH)的临床病理特点及鉴别要点。方法对1例cBFH进行临床病理分析及免疫组化研究,同时复习相关文献。结果肿瘤位于鼻背部,呈灰白、鱼肉样,镜下突出的特征是密集的梭形细胞呈束状排列,胞质嗜酸性,核分裂象4/10HPF,累及真皮网状层。免疫组化vimentin和CD68阳性。结论该瘤临床复发率高达26%,组织学特征为高度富于细胞、束状结构、有较多的核分裂象,需要与平滑肌肉瘤及皮肤隆突性纤维肉瘤鉴别。

Celluar benign fibrous histiocytoma: a case report and review of literature

Purpose To study the clinical pathologic characteristics and differential diagnosis of celluar benign fibrous histiocytoma (cBFH). Methods One case of cBFH was studied by clinical data analysis, light microscopy and immunohistochemistry. Results The tumor was located in the skin of the nose-back. The lesions were gray-white in colour, tender in consistency. Distinctive histologic features were a commonly fascicular growth pattern, predominance of eosinophilic spindle cells with oval vesicular nuclei and a small eosinophilic nucleolus,a moderate mitotic rate(4/10 HPF),and extension to the deep reticular dermis. Immunohistochemical studies found vimentin and CD68 positivity. Tests for CD34,desmin,S-100 were negative. Conclusions About one fourth (26%) of the tumor recur locally from 3 months to 3 years after excision. The cBFH should be distinguished from dermatofibrosarcoma protuberans and leiomyosarcoma,with which many of these cases were initially confused.
