急性脊髓炎的MRI诊断及鉴别诊断

目的总结急性脊髓炎的MRI表现及鉴别诊断要点。方法收集经影像与临床确诊的15例急性脊髓炎患者的临床及MRI影像资料，分析MRI所见的特征及其与临床预后的关系。结果颈段9例，胸段6例。8例病变范围超过3个脊髓节段；6例有不同程度的脊髓肿胀。脊髓肿胀者预后差，与无脊髓肿胀者比较有显著差异（P〈0．05）。结论急性脊髓炎有特征性MRI表现，结合典型的临床表现可对本病作出正确的诊断和鉴别诊断。

MRI diagnosis and differential diagnosis of the acute myelitis

Objective To summarize MRI findings of the myelitis and its gist of differential diagnosis.Methods Clinical-and MR imaging data of 15 patients with clinically proved myelitis were collected and correlated with the prognosis.Results Among 15 cases,acute myelitis of the cervical segment was found in 9,that of the thoracic segment in 6;that in 8 cases exceeded 3 spinal segments;that in 6 cases associated with spinal swelling in different extent.Compared with the patients without spinal swelling,the patients with spinal swlling showed poor prognosis,there was a significant difference in the both groups (P < 0.05).Conclusion Acute myelitis has characteristic MRI findings,while the integration of both MR imaging and typical clinical manifestations can accurately diagnose and differentiate this discase.
