小肾癌的超声造影与增强CT对照研究

[目的]探讨超声造影对直径≤3cm小肾癌的诊断价值。[方法]回顾性分析70例经手术病理证实的小肾癌病例的实时超声造影和增强CT资料,比较两种方法中小肾癌的灌注特征、准确性及对坏死灶和假包膜的显示率。[结果]肾细胞癌在超声造影和增强CT中的灌注特点表现为"快进快退"者分别为74.29%和78.57%。超声造影鉴别诊断小肾癌的准确性为84.29%,对假包膜和坏死灶的显示率分别为61.43%、55.71%;增强CT诊断小肾癌的准确性为87.14%,对假包膜和坏死灶的显示率分别为64.29%、58.57%。超声造影和增强CT对小肾癌诊断的准确性无统计学差异(P>0.05),两者对假包膜及坏死灶的显示率亦无明显统计学差异(P>0.05)。[结论]超声造影可作为诊断小肾癌的一种重要方法,与增强CT起到相辅相成的作用。

Contrast-enhanced Ultrasonography of Small Renal Cell Carcinomas:Compared to Contras t-enhanced CT

[Objective]To evaluate the value of clinical application between contrast—enhanced sonography and contrast—enhanced CT in diagnosis of small renal cell carcinomas.[Methods]Seventy patients with small renaltumors were detected respectively by contrast—enhanced ultrasound and contrast—enhanced CT.Enhancement patterns and perfusion characters of renal cell tumors were compared.All patients with small renal cell carcinomas were proved by operation and pathology.[Results]Enhancement patterns of contrast—enhanced sonography and contrast-enhanced CT in the majority of small renal cell tumors showed as presence of fast wash—in and fast wash—off were 74.3%(52/70) and 78.65(55/70),respectively. In the differentiating of renalmalignant tumors and benign tumors,the accuracy of contrast—enhanced sdnography was 84.3%(59/70),respectively, that of contrast-enhanced CT was 87.1(61/70).There were no differences of the accuracy in the differentiating of renal cell carcinomas. [Conclusions]Contrast enhanced sonography has become one of important methods to diagnosis small renal cell carcinomas.
