理肠汤内服联合溃疡灵灌肠治疗脾虚湿热型左半结肠溃疡性结肠炎临床研究

目的:观察理肠汤内服联合溃疡灵灌肠对脾虚湿热型左半结肠溃疡性结肠炎患者中医症候疗效、改良Mayo活动指数及血清C-反应蛋白、血沉、血小板、D-二聚体水平的影响,评定其综合疗效。方法:将符合标准的66例溃疡性结肠炎患者,按数字表法随机分成治疗组和对照组,每组各33例。其中治疗组予理肠汤内服联合溃疡灵,对照组予服用美沙拉嗪缓释颗粒,2组均按3个月为1个疗程,服完1个疗程后由同一位内镜医师复查结果,比较2组的中医症候疗效、改良Mayo活动指数及血清C-反应蛋白、血沉、血小板、D-二聚体水平,评定其综合疗效。结果:2组在中医症候疗效方面,治疗组的总有效率(87.5%)高于对照组(71.0%),差异具有统计学意义(P<0.05);2组改良Mayo活动指数均显著下降(P<0.01),治疗组除了内镜表现方面,其余各指标均低于对照组(P<0.05);2组各项血清C-反应蛋白、血沉、血小板、D-二聚体均明显下降(P<0.01),治疗组除了D-二聚体,其余各指标均低于对照组(P<0.05)。结论:理肠汤口服配合溃疡灵灌肠能有效缓解脾虚湿热型左半结肠UC患者症状,降低疾病活动指数,减轻炎症反应,改善微循环,对结肠粘膜有很好的保护及修复作用,具有确切的临床疗效。

Clinical Study on Treatment of Left Ulcerative Colitis of Spleen Deficiency and Damp-heat Type by Lichang Decoction Combined with Ulcer Ling

Objective:To observe the effect of taking Lichang decoction combined with ulcer ling enema on the symptoms of traditional Chinese medicine,modified Mayo activity index and serum C-reactive protein,erythrocyte sedimentation rate,platelet and D-dimer levels in patients with spleen-deficiency damp-heat ulcerative colitis Its comprehensive efficacy.Methods:66 patients with ulcerative colitis who met the standard were randomly divided into a treatment group and a control group according to the digital table method,with 33 cases in each group.Among them,the treatment group was given Lichang decoction combined with ulcer ling,and the control group was given Mesalazine sustained-release granules.Both groups took 3 months as a course of treatment,and the same endoscopist reviewed the results after taking a course of treatment The curative effect of traditional Chinese medicine symptom,modified Mayo activity index and serum C-reactive protein,erythrocyte sedimentation rate,platelet and D-dimer levels of the two groups were compared to evaluate their comprehensive curative effect.Results:In terms of the efficacy of TCM symptoms,the total effective rate of the treatment group(87.5%)in the 2 groups was higher than that in the control group(71.0%),the difference was statistically significant(P<0.05);the modified Mayo activity index in both groups decreased significantly(P<0.01),except for the endoscopic performance of the treatment group,all other indicators were lower than those of the control group(P<0.05);the serum C-reactive protein,erythrocyte sedimentation rate,platelets,and D-dimer of the 2 groups were significantly reduced(P<0.01),except for D-dimer in the treatment group,all other indexes were lower than those in the control group(P<0.05).Conclusion:Oral administration of Lichang decoction combined with ulcer ling can effectively relieve the symptoms of UC patients with spleen deficiency,dampness and heat,reduce the disease activity index,reduce inflammation,improve microcirculation,and have a good protection and repair effect on the colonic mucosa.Clinical efficacy.
