腹腔镜联合胆道镜治疗胆总管结石的临床效果分析

目的:探讨腹腔镜联合胆道镜治疗胆总管结石的临床疗效。方法:回顾分析2012年1月至2016年1月为208例胆囊结石合并胆总管结石患者进行手术治疗的临床资料,其中107例行腹腔镜联合胆道镜手术(腔镜组),101例行开腹手术(开腹组),观察比较两种手术方法的临床治疗效果。结果:腔镜组手术时间、术中出血量、术后疼痛感、肛门排气时间、引流管拔管时间、住院时间均优于开腹组,差异有统计学意义(P0.05);住院总费用、切口感染率、结石残余率两组相比差异无统计学意义(P0.05)。结论:腹腔镜联合胆道镜治疗胆总管结石的手术效果、治疗费用、术后主要并发症(切口感染率、结石残余率)与传统开腹手术相似,但腹腔镜手术具有患者创伤小、康复快的优点,临床应用价值较高,值得推广应用。

Clinical effect analysis of laparoscopy combined with choledochoscopy in the treatment of choledocholithiasis

Objective:To investigate the clinical effect of laparoscopy combined with choledochoscopy in the treatment of chole -docholithiasis .Methods:A retrospective analysis was made on the clinical data of 208 patients who suffered from choledocholithiasis with cholecystolithiasis and underwent surgery from Jan .2012 to Jan.2016.107 patients of them received laparoscopy combined with choledochoscopy ( laparoscopy group ) ,and the other 101 patients received traditional surgical treatment ( laparotomy group ) .The data of the patients were collected to compare the effects of the two groups .Results:The operation time ,intraoperative bleeding volume ,post-operative pain,anal exhaust time,extubation time of drainage tube and hospitalization time of the laparoscopy group were superior to those of the laparotomy group (P<0.05).But there were no significant differences between the two groups in total cost of hospitaliza -tion,infection rate of incision,residual stone rate ( P<0.05).Conclusions:Compared with traditional surgical method ,the clinical effect,the total cost and postoperative main complications ( rate of incision infection ,residual stone rate ) of laparoscopy combined with choledochoscopy in the treatment of choledocholithiasis with cholecystolithiasis are equivalent .But it has the advantages of fewer trauma and faster recovery ,so it is worthy of widely clinical application .
