单孔腹腔镜与多孔腹腔镜手术治疗小儿卵巢囊肿的对比研究

目的:比较经脐单孔腹腔镜与传统多孔腹腔镜小儿卵巢囊肿剥除术的临床疗效。方法:选取收治的48例卵巢囊肿剥除手术的患儿,随机分为观察组与对照组,每组24例,观察组行经脐单孔腹腔镜手术,对照组行传统多孔腹腔镜手术,对比两组患儿手术效果及美观满意度。结果:手术均顺利完成,两组患儿术后住院时间、术中出血量差异无统计学意义(P0.05)。观察组与对照组手术时间平均为(26.8±7.2)min与(40.6±9.4)min(P0.05);术后排气时间平均(19.8±5.6)h与(25.7±6.9)h(P0.05);术后美容满意度评分为(4.2±0.8)分与(3.3±0.7)分(P0.05)。结论:单孔腹腔镜手术治疗小儿卵巢良性囊肿安全、可行,手术时间短,胃肠功能恢复快,并能达到更好的美容效果,值得临床推广。

Comparison of single-port laparoscopic surgery and multi-port laparoscopic surgery in pediatric ovarian cysts

Objective:To compare the clinical effect of transumbilical single-port laparoscopic surgery and traditional multi-port laparoscopic surgery in pediatric ovarian cysts .Methods:Forty-eight children with ovarian cyst surgery were selected ,and randomly di-vided into observation group and control group with 24 children in each group .The observation group received transumbilical single-port laparoscopic surgery ,the control group was given traditional multi-port laparoscopic surgery .Clinical effect and aesthetic satisfaction were compared between the two groups .Date of these children were analyzed .Results:All cases were operated successfully .The differ-ences of blood loss and postoperative hospital stay between the two groups were not statistically significant (P>0.05).There were differences between observation group and control group in the average operation time [(26.8 ±7.2) min vs.(40.6 ±9.4) min],the average postoperative exhaust time [(19.8 ±5.6) h vs.(25.7 ±6.9) h],the average aesthetic satisfaction score [(4.2 ±0.8) vs. (3.3 ±0.7)].Conclusions:The single-port laparoscopic surgery is safe and feasible for resection of benign ovarian cysts in children , with shorter operative time ,faster recovery of gastrointestinal function ,and better cosmetic outcome .It is worth further clinical applica-tion.
