膀胱癌112例综合治疗分析

目的：探讨膀胱癌的综合治疗。方法：选取1998年1月-2003年8月收治的112例膀胱癌患者,男69例,女43例,平均年龄56岁。采用手术方法有三种,一为经尿道膀胱肿瘤切除术（TUR-BT）,二为膀胱部分切除术,三为根治性膀胱切除术。术后采用膀胱灌注化疗与全身化疗。结果：61例Ta-T2期行TUR-BT术患者术后5年复发率为21.3%,5年死亡率为9.8%。31例T1~T2期行膀胱部分切除患者术后5年复发率为22.6%,5年死亡率为16.1%。20例T1~T3膀胱根治切除术后患者5年复发率为30%,5年死亡率为35%。结论：膀胱癌易发现,早诊断,早治疗,根据患者肿瘤不同分期病理类型及身体状况制订相应的治疗方案。术后加强复查随访,可有效降低患者复发率,死亡率。

The analysis of the comprehensive treatment of 112 patients with bladder cancer

Objective To explore the comprehensive treatment of bladder cancer.Method This thesis selected 112 patients of bladder cancer with the average age of 56-year-old as participants,including 69 males and 43 females,all of whom were the author's patients from January 1998 to August 2003.Three operation methods were adopted: the first one was transurethral resection of bladder tumor(TUR-BT);the second was partial cystectomy of bladder;the third was overall cystectomy.After operation,bladder chemotherapy and systemic chemotherapy was followed.Results During the five years,the relapse rate was 21.3% with the 61 patients of Ta～T2 with TUR-BT and the mortality was 9.8%;the relapse rate was 22.6% with the 31 patients of T1~T2 with partial cystectomy of bladder and the mortality was 16.1%;however,the relapse rate was 30.0% with the 20 patients of T1~T3 with overall cystectomy and the mortality was 35.0%.Conclusion It is essential to diagnose bladder cancer early.Based on different tumors,appropriate treatment should be arranged in line with different types of pathology and different physical conditions.Then countercheck and follow-up survey can be followed to reduce efficiently the relapse rate and the mortality in patients.
