糖尿病肾病合并非糖尿病肾病临床病理分析

目的探讨糖尿病肾病合并非糖尿病肾病的发生率和临床病理特征。方法回顾性分析1995—2005年诊断为2型糖尿病、糖尿病肾病的75例患者的临床资料。结果糖尿病肾病患者中有14．7％合并非糖尿病肾病。非糖尿病肾病IgA肾病为45．4％，其他依次为膜性肾病27、3％，间质性肾炎18、2％，高血压肾损害9．1％。患者组织学呈现上述肾脏疾病相应的病理特征。结论对于临床表现为蛋白尿和（或）血尿的患者，不论有无眼底病变，肾活检都是一项排除非糖尿病肾病的重要手段。

Pathological analysis of type-2 diabetes nephropathy combined with non-diabetic renal disease

Objective To investigate the incidence and pathological characteristics of diabetes nephropathy combined with non-diabetic renal disease(NDRD).Methods Seventy-five patients with type-2 diabetes and diabetes nephropathy were studied retrospectively.Results In the 75 diabetic patients,11(14.7%) were combined with NDRD(8 males and 3 females).In the NDRD cases,IgA nephropathy accounted for 45.4%,followed by membrane nephropathy for 27.3%,interstitial nephritis for 18.2%,renal impairment induced by hypertension for 9.1%.The pathological characteristics paralleled the clinical manifestations of the renal disease.Conclusion Renal biopsy is an important approach to exclude NDRD for this kind of patients with manifestations of proteinuria and/or hematuria,no matter whether these patients implicate retinopathy or not.
