硬膜外腔注射确炎舒松治疗强直性脊柱炎

目的　探讨硬膜外腔注射确炎舒松对强直性脊柱炎 (AS)患者的治疗作用。方法　 6 4例AS患者按入院先后顺序被分为两组 ,治疗组采用硬膜外腔注射确炎舒松的方法 ;对照组采用腰部痛点注射确炎舒松的方法。每周注射 1次 ,共注射 2次。观察首次注射后到症状开始缓解时间 ,注射 10d后腰痛缓解积分 ,Schober试验增加的距离 ,晨僵改善积分 ,并经t检验。结果　上述观察指标中腰痛缓解积分、晨僵缓解积分治疗组 (7 2± 1 6 ,7 0± 1 3)明显高于对照组 (3 9± 1 2 ,4 0± 1 1) ,两组相差有显著性意义 (P <0 0 0 1) ;注射后治疗组症状改善明显比对照组快 (P <0 0 0 1) ;治疗组腰部活动度的改善好于对照组 (P <0 0 1)。结论　硬膜外腔注射确炎舒松可以快速缓解强直性脊柱炎患者腰背部症状 ,减轻疼痛 ,改善活动范围 ,防止或纠正脊柱畸形。

Patients with ankylosing spondylitis were treated by Triamcinolone acetonide injection into epidural cavity

Objective To investigate the effect of Triamcinolone acetonide injected into epidural cavity of patients with ankylosing spondylitis Methods 64 cases with ankylosing spondylitis were divided into two groups by hospitalized order of sequence.The patients in the treated group were injected Triamcinolone acetonide into epidural cavity,the patients of the control group were injected same drug in pain point in waist.Two groups were injected again after a week.The pain-relieving time was observed,and pain scales and morning-stiff points were recorded after ten days of the injection.Results Compared with the control group,the pain scales and morning-stiff points were higher in the treated group(P<0.001).The symptoms improved in the treated group.Conclusion The method that Triamcinolone acetonide were injected into the epidural cavity of patients with ankylosing spondylitis can relieve back pain,improve the back symptoms and mobility.
