血清C反应蛋白与前白蛋白在早产儿感染早期诊断中的应用

目的观察分析血清C反应蛋白与前白蛋白二者在早期诊断早产儿感染中的应用。方法所选150例患儿均为笔者所在医院新生儿科2010年2月～2011年10月住院的早产儿,其中男婴84例,女婴66例;胎龄27～35周,平均(31.6±2.6)周,出生时体重820～2950g,平均(1960±710)g,雷白杆菌阳性的患儿1例,阴沟杆菌阳性1例。CRP浓度作为特异性最高诊断指标,50%PA次之;而75%PA是敏感性最高诊断指标,但特异性最差。结果结合分析血清CRP和PA水平(960±710)g。所有病例的细菌感染情况均依据本试验诊断标准,150例早产儿其中发生院内感染的患者19例,肺炎患者13例,败血症患者6例。结论血清C反应蛋白与前白蛋白二者的应用对早产儿感染早期的诊断及治疗具有重要的临床协助诊断意义,值得临床工作中借鉴应用。

Application of serum C-reactive protein and prealbumin in early diagnosis of premature children with infection

Objective To observe and analyze the application of serum C-reactive protein and prealbumin in the early diagnosis of premature children with infection. Methods All the 150 selected patients were premature children hospitalized in the neonatology department of our hospital from February 2010 to October 2011,of whom 84 were male and 66 were female.The gestational age ranged from 27 to 35 weeks,with an average gestational age of(31.6±2.6)weeks;the birth weight ranged from 820 to 2 950 g,with an average weight of(1 960±710)g. One patient was klebsiella-positive and one was enterobacter cloacae-positive.The CPR concentration served as the diagnostic indicator with the highest specificity,followed by 50%PA;the 75%PA served as the diagnostic indicator with the highest sensitivity and the lowest specificity. Results The serum CRP and PA levels were analyzed.All the bacterial infection conditions were based on the diagnostic criteria of this experiment.Of the 150 premature children,19 had nosocomial infection,13 had pneumonia and 6 had sepsis. Conclusion Serum C-reactive and prealbumin are of important auxiliary clinical diagnosis value in the early diagnosis of premature children with infection,thereby worthy of application in clinical work.
