退变性腰椎管狭窄症手术治疗的临床分析

目的探讨退变性腰椎管狭窄症手术治疗的临床诊断和治疗方法。方法选取笔者所在医院收治的退变性腰椎管狭窄症病例90例,对其临床资料进行回顾性分析。结果手术时间1~4h,术中出血量为230~1350mL,术后随访9~36个月,X线片复查显示没有椎弓根钉误置、断钉以及拔出等情况发生,大部分患者症状和体征均得到改善,其中单纯退变性腰椎管狭窄手术有效率明显高于复杂性腰椎管狭窄手术,而并发症要低于复杂性腰椎管狭窄手术。结论脊髓造影诊断退变性腰椎管狭窄症具有较高的准确性,按照有效减压和限制破坏的原则,严格掌握手术适应证,采用手术治疗退变性腰椎管狭窄症可以取得较好的疗效,且可有效降低并发症发生。

Clinical analysis of degeneration of lumbar spinal stenosis surgery

Objective To investigate the clinical diagnosis and treatment of back degeneration of lumbar spinal stenosis surgery. Methods 90 cases of degenerative lumbar spinal stenosis cases in our hospital were setected and retrospectively analyzed the clinical data. Results The operative time was 1-4 h,intraoperative blood loss of 230-1350 mL,patients were followed up for 9-36 months,the X-ray examination showed no pedicle screw misplacement,breakage and pull out from happening,in some cases the signs and symptoms improved,of which only degenerative lumbar spinal stenosis was significantly higher than the complexity of the lumbar spinal canal stenosis,and complications was lower than the complexity of the lumbar spinal canal stenosis. Conclusion Myelography diagnosis of degenerative lumbar spinal stenosis with high accuracy,in accordance with the principle of effective decompression and to limit the damage,with strict control of the surgical indications,surgical treatment of degenerative lumbar spinal stenosis can get a good effect,and can effectively reduce the complications.
