摘    要:目的 探讨SonoVue子宫输卵管超声造影(SonoVue-HyCoSy)评价输卵管通畅性的可行性.方法 对80例不孕症患者(原发不孕33例,继发不孕47例)进行SonoVue-HyCoSy检查,实时观察造影剂在宫腔及输卵管腔内流动过程,判断输卵管的通畅性.随机对20例患者行腹腔镜下美兰通染液试验(CLP)进行对照.结果 两种检查结果差异无统计学意义(χ2=0.21, P=0.65).以CLP结果为诊断标准,SonoVue-HyCoSy评价输卵管通畅的敏感性、特异性、阳性预测值、阴性预测值分别为92.00%、100%、100%和88.23%,诊断输卵管阻塞部位准确率为70.59%,且无明显并发症.结论 SonoVue可用于子宫输卵管超声造影检查,可明确输卵管的通畅性并可同时提示输卵管阻塞部位,该方法准确、简便易行、无创,费用低,具有推广应用前景.

Abstract:Objective To evaluate SonoVue-hysterosalpingo-contrast sonography (SonoVue-HyCoSy) in the assessment of tubal patency. Methods Eighty patients,33 with primary infertility and 47 with secondary infertility underwent SonoVue-HyCoSy. Real-time assessment of tubal patency was conducted by monitoring motion of contrast agent in the endometrial cavity and tubal lumen. Twenty of the patients who underwent laparoscopy and dye test were randomly selected as the control group. Results The sensitivity,specificity,positive predictive value and negative predictive value of SonoVue-HyCoSy was 92.00%,100%,100% and 88.23%,respectively,without statistically significant difference compared with the results of laparoscopy and dye test. The accuracy of SonoVue-HyCoSy in diagnosing the tubal occlusive position was 70.59%,and no significant complications occurred. Conclusion SonoVue-HyCoSy is reliable,non-invasive,reproducible and inexpensive. This method is able to demonstrate not only fallopian tube occlusion,but also the location of occlusion,which could benefit for women with infertility.
