前降钙素在儿科临床的应用

摘    要:目的探讨血清前降钙素(PCT)对细菌感染的诊断价值。方法采用前瞻性研究方法,根据病史、临床表现及实验室检查结果对189例小儿作出诊断,将其分为正常对照组(40例)、细菌感染组(51例)、病毒感染组(49例)和自身免疫疾病组(49例)。比较各组入院时、入院后1周血清PCT、C-反应蛋白(CRP)水平和血白细胞(WBC)计数值。结果入院时血清PCT水平细菌感染组高于正常对照组、病毒感染组和自身免疫疾病组(均P〈0.05);血清CRP水平、血WBC计数细菌感染组、自身免疫疾病组高于病毒感染组及正常对照组(均P〈0.05),但细菌感染组与自身免疫疾病组比较,差异无统计学意义(均P〉0.05)。入院后1周,血清PCT水平细菌感染组高于正常对照组、病毒感染组和自身免疫疾病组(均P〈0.05);血清CRP水平、血WBC计数细菌感染组、自身免疫疾病组高于病毒感染及正常对照组(均P〈0.05),但细菌感染组与自身免疫疾病组比较,差异无统计学意义(均P〉0.05)。结论血清PCT对鉴别细菌感染优于血WBC计数及血清CRP,是诊断细菌感染性疾病较为敏感的指标。

Abstract:Objective To evaluate the diagnostic value of serum procalcitonin(PCT) for bacterial infection.Methods A prospective study was performed to diagnose bacterial infection in 189 children based on medical history,clinical situation and laboratory test results.These children were divided into four groups: normal control group(40 cases),bacterial infection group(51 cases),viral infection group(49 cases) and autoimmune system disease group(49 cases).Serum PCT and C-reactive protein(CRP) levels and white blood cell(WBC) count were measured at admission and after 1 week.Results Serum PCT levels in bacterial infection group were higher than those in other three groups at admission and after 1 week(P0.05).Futhermore,CRP levels and WBC count in bacterial infection group and autoimmune system disease group were higher than those in other two groups(P0.05),but no significant differences were found between acterial infection group and autoimmune system disease group(P0.05).Conclusion PCT may be a more sensitive marker than WBC and CRP in diagnosing bacterial infection.
