摘    要:
目的比较后腹腔镜下肾盂成形术与开放肾盂成形术的疗效。方法回顾分析同一组医生完成的80例单纯性肾盂输尿管连接部(UPJ)狭窄患者,其中后腹腔镜下与开放手术各40例,均采用利尿性肾图、IVU、磁共振尿路造影术(MRU)等检查确诊。2组患者年龄、性别、左右侧别等差异均无统计学意义(P〉0.05),分析比较2组患者手术时间、术中出血量、术后并发症及平均住院时间的差异。结果 2组患者均顺利完成手术,术中术后未发生外科并发症。经后腹腔镜组和经腰部斜切口组在术中失血量、引流管放置时间及住院时间方面,两组间指标差异有统计学意义(P〈0.05)。结论后腹腔镜下肾盂成形术有与开放手术同样高的成功率,疗效肯定,与开放手术相比创伤小,术中出血少,术后住院时间短,恢复快,患者痛苦小,可能成为治疗UPJO的标准方法。

Abstract:
Objective To comparison of the efficacy of laparoscopic pyeloplasty and open pyeloplasty surgery. Methods Retrospective analysis of the same group of doctors who had completed 80 cases of simple uretero pelvic junction ( UPJ) stenosis was performed in 40 cases of each group. Diuretic renograph、IVU、magnetic resonance urography ( MRU) were used to confirm the diagnosis,there was no statistical significance ( P 0. 05) in age,gender and each side of the two groups,surgery time、blood loss、postoperative complications and the average length of stay differences were analyzed of the two groups. Results Surgery were successfully completed in 2 groups,there was no intraoperative and postoperative occurrence of surgical complications. Difference in surgery time、blood loss and storage of drainage tube between the two groups has statistically significant ( P 0. 05) . Conclusion Laparoscopic pyeloplasty surgery has the same high success rate compared with open surgical,less trauma,less blood loss,shorter postoperative hospital stay,quicker recovery,little pain, which may be the standard method of the treatment of UPJO.
