人工流产术中不同镇痛方法的疗效观察

目的:探讨门诊无痛人工流产术的最佳镇痛方法。方法:选取门诊就诊早孕妇女240例,随机分为三组。A组:术前10min静脉缓慢注射地西泮10mg和宫颈注射1%利多卡因5ml;B组:术前2min静脉推注芬太尼0.1mg,术中缓慢静脉推注丙泊酚2.5mg/kg;C组:不采取任何干预措施。比较三组镇痛效果及不良反应。结果:B组镇痛优于A组,A组镇痛效果优于C组;A、B两组不良反应发生率均低于C组。结论:利多卡因加安定用于人工流产效果明显,不良反应小,适合在基层医院门诊使用;小剂量丙泊酚复合芬太尼麻醉效果好,对呼吸循环无抑制,无人工流产综合征发生,适合有专职麻醉师的医院使用。

Effect of different analgesia on induced abortion

Objective To explore the best analgesia method of clinic induced abortion.Method Total 240 earlier pregnant women were divided randomly into three groups：group A：diazepam（10 mg）slowly injection through vein 10 min before operation and 1% xylocaine（5 ml）injection in cervix;group B：fentanyl（0.1 mg）slowly injection through vein 2 min before operation and propofol（2.5 mg/kg）slowly injection through vein during operation;group C：no intervention was done.Analgesia and side effect were contrasteded.Results Analgesia of group B had an advantage over group A and analgesia of group A was better than group C;incidences of side effect of group A and B were lower than group C.Conclusion Xylocaine combined with diazepam has obvious effect on induced abortion and is suitable to base hospital;small dosage propofol combined with fentanyl has good anesthesia effect and has not respiratory depression and induced abortion syndrome,so it is a good choice of the hospital which has full-time anesthetist.
