舌下免疫治疗常年性变应性鼻炎临床研究

目的观察标准化粉尘螨变应原疫苗舌下含服免疫治疗常年性变应性鼻炎的临床疗效及安全性。方法82例均对粉尘螨过敏的常年性变应性鼻炎患者，给予标准化粉尘螨变应原疫苗舌下含服免疫治疗1年，观察免疫治疗过程中局部及全身不良反应发生情况，并通过临床症状评分及鼻结膜炎相关生活质量问卷进行评价其疗效。结果局部不良反应有5例（6．1％），表现为口腔局部瘙痒不适；全身不良反应有10例（12．2％），包括胃肠道反应、荨麻疹、鼻炎症状加重等，主要发生于剂量递增阶段，均为轻至中度反应，未发生过敏性休克等严重不良反应。临床症状及生活质量均得到显著性改善（P〈0．01），治疗总有效率为84．1％（69／82）。结论舌下含服途径进行特异性免疫治疗常年性变应性鼻炎具有良好的临床疗效及安全性，值得推广应用。

Clinical study of perennial allergic rhinitis with sublingual immunotherapy

Objective To observe the efficacy and safety of sublingual immunotherapy in treating perennial allergic rhinitis. Methods 82 patients with perennial allergic rhinitis caused by Dermatophagoides farinae were enrolled in this self - contrast study. Clinical efficacy was evaluated by symptom scores visual analogue scale and rhinoconjunctivitis quality of life questionnaire after one year of sublingual immunotherapy and compared with pre - treatment scores. Results Oral itching as- sociated with drop intake was reported by 5 subjects （6. 1% ） in the sublingual immunotherapy. There were also 10 patients （12. 2% ） who experienced systemic reactions in the sublingual immunotherapy,including gastrointestinal symptoms, urticaria, and rhinitis exacerbations. Systemic reactions were mainly observed in patients during the up - dosing phase of treatment. There were mild or moderate and no anaphylactic shock occurred. After therapy for one year, clinical symptoms and quality of life were significantly improved （P 〈 0.01 ）. There were effective after treatment in 84. 1% cases （69/82）. Conclusion Sublingual immunotherapy with dermatophagoides farinae suffering from perennial allergic rhinitis, so it was worth spreading.
