碘苯酯肝动脉栓塞的动物实验

用健康杂种小猪进行碘苯酯肝动脉栓塞(hepatic arterial embolization,简称HAE),探讨碘苯酯肝动脉栓塞的有关并发症及相对危险性。结果表明,碘苯酯可以进入肺循环产生肺油栓,并引起相应病理改变,甚至致死,还可以引起正常肝组织梗死,在梗死区较长时间残留,甚至产生X线的假肿瘤征象。

A STUDY ON THE COMPLICATIONS OF HEPATIC ARTERIAL EMBOLIZATlON WITH MYODIL IN PIGS

Complications of hepatic arterial embolization(HAE) with myodil were studied in pigs. It was shown that HAE with myodil led to pulmonary oil embolism. The mortality rate in pigs undergoing HAE with myodil of 1 ml/kg body weight was 50%. Though there was no death in pigs undergoing HAE with myodil of 0.25ml/kg body weight, oil emboli and irreversible lesions could still be found in their lungs. Myodil HAE resulted in infarction of the liver in both groups of pigs. Stagnation of myodil in the infarct area produced false-positive radiopaque shadows mimicing the signs of malignant space occupied lesions as seen in iodized roentgenography through the same route for diagnostic purpose. Based on the findings mentioned, care must be taken to those patients with poor pulmonary function in selecting candidates for this procedure. Monitoring of the procedure under video supervision is recommended.To avoid extensive damage of the normal liver tissue, highly selective catheterization of the hepatic artery is suggested. Precaution should be taken in the diagnosis of liver tumors with iodized oil roentgenography through the hepatic arterial route for the possibility of false positive signs
