输卵管妊娠腹腔镜保守性手术后持续性异位妊娠的临床分析

目的总结输卵管妊娠腹腔镜保守性手术后持续性异位妊娠(PEP)的发生原因及预防和处理措施。方法回顾性分析我院2008年1月至2010年8月因输卵管妊娠而行腹腔镜保守性手术的患者425例。根据术后是否发生持续性异位妊娠分A组(正常组)和B组(PEP组),比较两组术前、术中及术后的情况。结果术后发生持续性异位妊娠15例(3.53%);术前血人绒毛膜促性腺激素(HCG)水平、输卵管妊娠部位、黄体是否剥除,两组比较,差异有统计学意义(P<0.05)。结论 术前血HCG水平、输卵管妊娠部位及黄体是否剥除均与PEP的发生密切相关,术中和术后采用各种预防措施可减少其发生率。

Clinical analysis on persistent ectopic pregnancy after conservative laparoscopic surgery for patients with tubal pregnancy

Objective To study the causes,prevention and treatment measures for persistent ectopic pregnancy（PEP） after conservative laparoscopic surgery for patients with tubal pregnancy.Methods A retrospective analysis on clinical data of 425 cases of tubal pregnancy after conservative laparoscopic surgery in this hospital during January 2008 to August 2010 was carried out.According to whether postoperative occurrence of persistent ectopic pregnancy,all these patients were divided into group A（normal group） and group B（PEP group）,and their preoperative,intraoperative and postoperative situations were compared.Results Persistent ectopic pregnancy had been occurred in 15 cases（accounting for 3.53%）.There was statistically significant difference among preoperative serum HCG levels,corpus luteum stripped or not and the location of tubal pregnancy between these two groups（P0.05）.Conclusion The preoperative serum HCG level,whether corpus luteum stripped or not and location of tubal pregnancy are closely related to the occurrence of PEP,and the application of various preventive measures during operation and post-operation can reduce its incidence.
