甲状腺微小癌的超声诊断评价

摘    要:目的 探讨甲状腺微小癌的超声特点,以提高该病的诊断符合率.方法 回顾性分析经手术和病理证实的31例甲状腺良性小结节患者(良性对照组)和34例甲状腺微小癌患者(微小癌组)的二维声像图特征及彩色多普勒血流成像显示血流分布特点.结果 良性对照组39个结节中,超声诊断符合率为92.3%(36/39);微小癌组42个结节中,超声诊断符合率为90.5%(38/42),且声像图表现为边界不清晰、低回声、微钙化、颈部淋巴结转移及Ⅲ级血流信号的患者较良性对照组多,差异有统计学意义(P<0.05).结论 甲状腺微小癌的二维声像图表现具有一定特征性,再结合彩色多普勒血流成像及颈部淋巴结检测,能明显提高甲状腺微小癌的检出率.

Abstract:Objective To explore the sonographic features of thyroid microcarcinoma, and to improve the diagnostic coincidence of thyroid microcarcinoma. Methods 2D ultrasonography and color Doppler flow imaging characteristics of 31 patients with thyroid benign lesion (benign group) and 34 patients with thyroid microcarcinoma (malignant group) were analyzed retrospectively, these patients were all proved by operation and pathology. Results There were 39 nubbles in benign group, the diagnostic accuracy rate of ultrasonography was 92.3% (36/39). There were 42 nubbles in malignant group, the diagnostic accuracy rate of ultrasonography was 90.5% (38/42). The thyroid microcarcinoma ultrasonography showed the boundary was indistinct, low echo, microcalcification, metastatic lymphonode of necks and Ⅲ flow signal, which were more than that in the benign group. There was a significant difference between the two groups (P〈0.05). Conclusion 2D ultrasonic appearences of thyroid microcarcinoma have certain characteristics, combined with color Doppler flow imaging and cervical lymph node detection, which can improve significantly the detection rate of thyroid microcarcinoma.
