高剂量吡喃阿霉素的CTOP方案治疗非霍奇金淋巴瘤

目的　观察高剂量吡喃阿霉素组成的CTOP方案治疗非霍奇金淋巴瘤的疗效及不良反应。方法　 60例初治非霍奇金淋巴瘤随机分为两组 ,分别接受常规剂量吡喃阿霉素的CTOP(THP 40mg/m2 )和高剂量吡喃阿霉素的CTOP(THP 60mg/m2 )方案化疗至少 2个疗程 ,观察疗效及不良反应。结果　高剂量吡喃阿霉素的CTOP与常规剂量吡喃阿霉素的CTOP方案相比完全缓解率有所提高(3 6 6%vs 2 0 % ) ,达到完全缓解的时间也缩短 ,但差异均无显著性 (P >0 0 5 )。提高吡喃阿霉素的剂量强度后并未增加心脏毒性、脱发及骨髓抑制等不良反应的发生 ,两组的不良反应的发生率及严重程度相仿 ,差异无显著性。结论　高剂量吡喃阿霉素组成的CTOP方案治疗非霍奇金淋巴瘤可提高疗效而不加重不良反应 ,高剂量吡喃阿霉素可安全使用。

Treatment of Non-Hodgkin's Lymphoma with CTOP Comprising of High Dose THP

Objective To investigate the efficacy and side-effects of CTOP comprising of high dose THP in the treatment of non-Hodgkin's lymphoma.Methods 60 patients with chemonaive NHL received CTOP either comprising of high dose THP (n=30) or comprising of conventional dose THP (n=30).All these patients received 2 cycles chemotherapy at least.The response and side-effect were observed after they received chemotherapy.Results CTOP comprising of high dose THP achieved better complete remission rate than that of CTOP comprising of conventional dose THP (36.6 % vs 20%) ,Time to achieve complete remission also was shorten .There was no significant difference between these two protocols(P>0.05). As to the side-effect the CTOP comprising of high dose THP was similar to CTOP comprising of conventional dose THP .There was no significant difference between these two protocols (P>0.05) . Conclusions CTOP comprising of high dose THP could improve efficacy. On the other hand, chemotherapy induced toxicity was not significantly intensified when dose of THP increased.
