血清胱抑素C对多发性骨髓瘤肾损害的早期评估

摘    要:目的检测多发性骨髓瘤(MM)患者血清胱抑素C(Cys-C)水平的变化,探讨其对肾损害的早期评估意义。方法选择42例初发MM患者(MM组)和30例健康体检者(对照组),MM组按照国际分期标准(ISS)分期:I期9例,Ⅱ期23例,Ⅲ期10例。分别测定其血清Cys-C、肌酐(SCr)及β2-微球蛋白(β2-MG)水平,并比较。结果MM组血清Cys-C水平高于对照组(2.34±0.57)mg/L比(0.66±0.12)mg/L],且Cys-C水平随ISS分期的升高而升高I,Ⅱ,Ⅲ期分别为(1.77±0.38),(2.11±0.46),(3.14±1.07)mg/L],差异均有统计学意义(P〈0.05)。血清Cys-C水平与β2-MG水平以及SCr水平呈正相关(r=0.760,0.694,P〈0.05)。结论MM患者血清Cys-C水平上调,并与肾损害及肿瘤负荷有关;其可作为评价早期肾损害及肿瘤负荷的潜在指标。

Abstract:Objective According to the level of serum cystatin C (Cys-C)in patients with multiple myeloma (MM),to explore its significance of early assessment to renal damage. Methods To select 42 patients with MM( MM group ) and 30 healthy subjects (control group ), and measure serum Cys-C, creatinine (SCr) and β2-microglobulin ( β2-MG). Results The level of serum Cys-C in MM group was significantly higher than that in control group (2.34± 0.57) mg/L vs. (0.66 ± 0.12) mg/L] ,which increased with ISS stageI, Ⅱ, Ⅲ stage was( 1.77 ±0.38), (2.11 ±0.46), (3.14 ± 1.07) mg/L](P〈 0.05). Cys-C and β2-MG, renal function had a positive correlation (r = 0.760,0.694,P〈 0.05). Conclusion The increasing of Cys-C in MM patients is related to impaired renal function and tumor burden,which can be used as an early assessment indicators.
