胃癌高发区居民幽门螺杆菌感染的研究

目的：探讨长乐市胃癌高发区居民的幽门螺杆菌（HP）感染情况及治疗效果。方法：用ELISA法、幽门螺杆菌快速试片法及13C－尿素酶呼气方法检测HP感染。结果：胃癌高发区长乐市居民血清HP感染率为75．64％，男76．13％，女74．96％。幽门螺杆菌试片HP感染率为79．66％，男80．66％，女78．30％。性别经检验差异无显著性（P＞0．05）。二者的诊断方法灵敏度和特异性相比，幽门螺杆菌试片为高，分别为90．8％和81．7％比85．6％和68．2％，2300人中胃癌检出19人。正常胃粘膜居民占35．74％，胃癌疾病占64．26％，其中胃炎占69．35％。随着年龄增长胃癌疾病增多，胃粘膜正常居民相对减少。HP感染阳性者经铋剂三联药物治疗有明显转阴，其转阴率为67．36％，男69．66％，女64．49％。结论：长乐市胃癌高发与HP感染率有密切关系，并与胃癌前病变相关。

RESEARCH OF HELICOBACTER PYLORI INFECTION IN AREA OF HIGHER INCIDENCE OF GAS TRIC CANCER

Objective:To research the situation of helicobacter H pylori infection in area of higher incidence of gastric cancer in Changle city and observe therapy results.Methods:H.pylori infection was detected by ELISA method,H.pylori rapid test slide method and 13C-urease expiratory-test.Results:The infectious rate in serum of resident with Helicobacter pylori infection in Changle was 75 64%,in which male and female were 76 13% and 74 96%,respectively.The infectious rate in H.pylori test slide method was 79 66%,in which male was 80 66% and female was 78 30%.There was no significant difference in sex( P> 0 05).Between two kinds of methods the sensitivity and speciality was high in H.pylori rapid test slide method.19 patients were detected as gastric cancer among 2300 persons.Residents with normal gastric mucosa were 35 74%,gastric disease occupied 64 26%.Positive patients with H pylori infection could be cured after therapy by combining three drugs with bismuth,their turning negative rate was 67 36%,in which thoes of male and female were 69 66% and 64 49%,respectively.Conclusion:High incidence of gastric cancer in Changle city had a closed relation with Helicobacter pylori infection and related to gastric precancerous lesion.
