老年膀胱颈硬化症的诊断与治疗

目的 探讨老年膀胱颈硬化症的诊断方法和比较经尿道电切术、开放性手术的治疗效果。方法 对１９８４年５月～１９９９年５月经手术治疗的老年膀胱颈硬化症进行回顾性分析，以手术前后残祭余尿、尿流率、术后留置导尿时间、患者出院为评价指标。结果 ２２例开放性手术，１９例能自行排尿，其中２例术后仍有排尿困难。术后３ｗ１７例复查尿流率为６．９～２３．６ｍｌ／ｓ，１９例复查残尿，其中１２例无残余尿，４例残余尿量１５～

Diagnosis and treatment of neck sclerosis of urinary bladder of the elderly patients

Objective To improve the diagnosis and treatment of bladder neck sclerosis, To compare the therapeutic effects after transurethral electroscission and open surgery. Methods Form May 1984 to May 1999 the residual urine,urinary flowing rtae of 62 cases of bladder neck sclerosis during operation were studied retrospectively with indwelling catheter time,leaving hospital times evaluated also. Results Open operation was performd in 22 cases, 19 cases can urinate spontaneously with 3 cases failed. Urinary flowing rate was reexamined after operation three weeks in 17 cases. It was 6. 9-23. 6 ml/s,residual urine was reexamined after three weeks in 19 cases, there was not residual urine in 12 cases, with 15 - 20 ml in 4 cases, and 100 - 150 ml in 3 cases. Transurethral electroscissions of the bladder neck were performed in 40 cases, urination was fully clear after operation, urinary flowing rate was reexamined after three weeks in 31 cases,it was 17. 2-24. 6 ml/s and there was not residual urine in 32 cases,and 15 ml in 4 cases. Indwelling catheter time and leaving hospital time were longer in the open operation cases. Conclusions Vesicourethroscope is reliable for the diagnosis of bladder neck sclerosis and transurethral electroscission is the better therapeutic choice.
