tPSA和cPSA联合检测在前列腺癌诊断中的作用

目的:探讨总前列腺特异抗原(tPSA)和复合前列腺特异抗原(cPSA)联合检测在前列腺癌诊断中的作用。方法:选取我院收治的经临床及病理组织活检证实的前列腺癌(PCa)患者46例,前列腺增生(BPH)患者214例,采用化学发光法对两组患者血清tPSA和cPSA进行检测分析,评价联合检测tPSA和cPSA在前列腺癌诊断中的价值。结果:PCa组cPSA、tPSA阳性率显著高于BPH组(P<0.05),PCa组与BPH组之间的tPSA、cPSA值及cPSA/tPSA比值均存在着显著性差异(P<0.05);利用cPSA与cPSA/tPSA在诊断灰值区附近可以有效地区分BPH和PCa患者,联合使用cPSA、tPSA和cPSA/tPSA比值进行筛选诊断时,其临床诊断概率比单纯使用tPSA、cPSA或cPSA/tPSA比值更高。结论:前列腺癌诊断中联合检测tPSA和cPSA可以提高前列腺癌的诊断特异性,在诊断灰区内也具有很重要的临床意义。

Impact of combined testing of tPSA and cPSA in prostatic cancer diagnosis

Objective:To discuss the impact of combined testing of tPSA and cPSA in diagnosing prostatic cancer(PCa).Methods:A total of 46 PCa patients and 214 benign prostate hyperplasia(BPH) patients admitted in our hospital and clinically confirmed by pathological biopsy were enrolled.tPSA and cPSA in both groups were measured using chemiluminescent assay so as to evaluate the vale of combined testing of tPSA and cPSA in diagnosing PCa.Results:PCa group had significantly higher positive rates of tPSA and cPSA than BPH group(P<0.05),and statistical differences were observed between two groups in tPSA,cPSA values and cPSA/tPSA ratio(P<0.05).BPH and PCa patients could be distinguished efficiently in diagnostic grey zone with the help of cPSA value and cPSA/tPSA ratio.Combined application of tPSA,cPSA values and cPSA/tPSA ratio provided higher clinical diagnostic rate than tPSA,cPSA values or cPSA/tPSA alone.Conclusions:Combined testing of tPSA and cPSA can raise specificity in diagnosing prostatic cancer.It is with great clinical significance in diagnosis grey zone as well.
