株洲市2004-2008年剖宫产率相关因素分析与干预措施

目的探讨株洲市剖宫产率及剖宫产指征的变化,提出干预措施。方法选择株洲市160所助产医院住院分娩5年间剖宫产74 591例,分析剖宫产率及剖宫产指征变化。结果①2004-2008年剖宫产率呈逐年上升,阴道助产率逐年降低。②剖宫产各因素所占的比例发生明显变化,其中以社会因素、疤痕子宫上升最快。结论剖宫产率升高是受多种因素影响,因此需要提高产科质量,正确掌握剖宫产指征,实行导乐陪伴与无痛分娩,建立卫生行政监督机制,加强健康教育,优化医疗环境可降低剖宫产率。

Analysis on Related Factors to Cesarean Section Rate and Its Intervention Measures in Zhuzhou During the Period of 2004-2008

Objective To explore the related factors to cesarean section rate and the changes of indications for cesarean section in Zhuzhou,and to put forward the intervention measures. Methods Totally 74,591 cases of cesarean section were selected from 160 hospitals in Zhuzhou during the period of 2004～2008,the cesarean section rate and the changes of indications for cesarean section were analyzed. Results The cesarean section rate was gradually increased from 2004 to 2008,but the vaginal delivery rate was decreased year by year.The proportions of the related factors to cesarean section were changed significantly,among them,the social factors and uterine scar were increased rapidly. Conclusions The increase of cesarean section rate is influenced by many factors.Therefore,it is necessary to reduce the cesarean section rate through intervention measures,such as improving the quality of obstetrics,correctly understanding the indications for cesarean section,implementing conducted accompany delivery and painless childbirth,enhancing health administrative supervision,strengthening health education and optimizing medical environment.
