磁共振成像技术在宫颈癌分期及治疗中的应用价值研究

目的 探讨磁共振成像技术(MRI)在宫颈癌分期及治疗中的应用价值.方法 选取81例宫颈癌患者,由2名副主任医师进行妇科盆腔检查,共同决定分期及手术可行性.对于可行手术或者经先期治疗后可以手术者,在治疗前1周内完成盆腔MRI扫描.全部患者最终均行手术治疗,以术后病理结果为金标准,将MRI检查结果与之比对,并进行诊断试验.结果 IB期以前宫颈癌患者MRI扫描有阳性发现者仅为2/6,而IB期以上患者MRI扫描均有阳性特征,75例确诊74例.MRI诊断宫旁浸润、阴道受累及淋巴结转移的灵敏度和特异度分别为:100.00%和93.05%,63.64%和90.06%,66.67%和98.41%.按照国际妇产科协会(FIGO)分期标准,MRI分期准确度为75.00%.结论 MRI对IB期以上宫颈癌诊断准确率高,对于判断宫旁浸润、阴道受累及淋巴结转移的准确率较高,其分期准确性明显高于临床分期,MRI可以作为宫颈癌患者治疗前评估病变范围的理想工具.

Application of MRI in Staging and Treatment of Cervical Carcinoma
