降钙素原检测对儿科危重监护病房感染性疾病预后的评估价值

目的探讨血清降钙素原对儿科危重监护病房感染性疾病的诊断以及预后判断的临床价值。方法回顾性分析我院儿科在2010年1月至2012年12月期间收治的120例感染性疾病患儿的临床资料，按照患者不同感染类型将上述患儿分为两组，其中细菌感染组65例，病毒感染组55例，对所有患儿进行血清降钙素原检查以及细菌血培养检查，选择同期在我院进行体检的正常儿童50例，对所有儿童的检查结果进行分析。结果细菌感染组患儿血清降钙素原检测结果为阳性者53例，阳性率为81．54％；病毒感染组患儿血清降钙素原检测结果为阳性者4例，阳性率为7．27％；正常组儿童血清降钙素原检测结果为阳性者为4例，阳性率为8．00％。细菌感染组患儿降钙素原阳性率显著高于病毒感染组和正常组，差异均具有统计学意义（P〈0．05）；病毒感染组患儿降钙素原阳性率与正常组差异无统计学意义（P〉O．05）。结论血清降钙素原能够准确地反映患儿感染性疾病的程度，对于患儿疾病的诊断以及预后判断具有良好的临床应用价值，对于患儿疾病的诊断、治疗以及预后评估具有很好的指导意义，值得临床推广。

Evaluation of PCT detection in the prognosis of infectious disease in pediatric ICU.

Objective To explore the clinical value of serum PCT detection in the diagnosis and prognosis of infectious disease in pediatric ICU. Methods 120 cases of children with infectious disease in the Pediatric Depart- ment of our hospital from January 2010 to December 2012 were retrospectively analyzed. According to different types of infections, these cases were divided into two groups. There were 65 cases in bacterial infection group while 55 cas- es in viral infection. All of these children had taken serum PCT detection and examination of bacteria in blood culture. Besides, 50 cases of normal children went through health examination in our hospital at the same term were chosen as the control group. The inspection results of all children were analyzed. Results In bacterial infection group, there were 53 positive cases of PCT detection and the positive results rate was 81.54%. In viral infection group, there were 4 pos- itive cases of PCT detection and the positive results rate was 7.27%. In normal children group, there were 4 positive cases of PCT detection and the positive results rate was 8.0%. The positive rate of PCT detection in bacterial infection group was significantly higher than those in viral infection group and normal group （P〈0.05）. The positive rate of PCT detection in vi- ral infection group and normal groups had no significant difference （P 〉 0.05）. Condusion Serum PCT could accurately reflect the degree of infectious diseases in children and had a great clinical value and guiding significance in the diagnosis, treatment and prognosis of infectious disease in children, which is worthy of promotion.
