腹腔镜高选择性迷走神经离断加修补治疗胃十二指肠溃疡穿孔临床研究

摘    要:目的探讨腹腔镜穿孔高选择性迷走神经离断加修补术治疗胃十二指肠溃疡穿孔的应用价值。方法回顾性分析2004年1月~2007年1月本院128例胃十二指肠溃疡穿孔患者的临床资料。其中,胃穿孔60例,十二指肠穿孔68例。65例行腹腔镜高选择性迷走神经离断加穿孔修补术(研究组);63例行单纯腹腔镜穿孔修补术(对照组)。结果研究组手术时间较对照组增加,术后5年复发率较对照组低。结论腹腔镜穿孔高选择性迷走神经离断加修补术治疗胃十二指肠溃疡穿孔具有微创、安全、复发率低等特点,条件允许时建议作为首选治疗术式。

Abstract:Objective To investigate the application value of laparoscopic perforation high selectivity of vagotomy plus repair surgery in the treatment of gastroduodenal ulcer perforation.Methods The clinical data of 128 cases with gastric ulcer perforation from January 2004 to January 2007 was analyzed retrospectively.Of which,there were 60 cases of gastric per foration,68 cases of duodenal perforation.65 patients were given laparoscopic perforated high selectivity of vagotomy plus repair surgery(study group);63 patients were given single laparoscopic perforation repair surgery(control group).Results The operation time of study group was longer than that in control group,the recurrence rate of 5 years after the operation was lower than that in control group.Conclusion The treatment of laparoscopic perforation high selectivity of vagotomy plus repair surgery in gastroduodenal ulcer perforation has the characteristics of minimally invasive,safe,low recurrence rate,and so on.When it is available,it should be recommended as the first choice of procedure.
