婴儿期BMI峰值与6岁儿童超重/肥胖关联的出生队列研究

摘    要:
目的 探讨婴儿期BMI峰值与6岁儿童超重/肥胖的关联。方法 基于2013年建立的“马鞍山优生优育队列(Ma’anshan Birth Cohort,MABC)”,对队列儿童连续追踪随访至6岁、定期体格测量收集相关信息,绘制BMI生长曲线。多因素logistic回归分析婴儿期BMI峰值与6岁儿童超重/肥胖之间的关联。结果 婴儿期BMI峰值时点时的BMI水平男童高于女童,差异有统计学意义(18.56kg/m2 vs. 18.14kg/m2,t = 5.155,P<0.01),峰值年龄性别之间无统计学差异(7.10月龄 vs. 7.19月龄,t = Symbolm@@1.084,P>0.05)。婴儿期峰值时的BMI水平高是6岁儿童超重/肥胖的危险因素(超重OR=1.58, 95%CI: 1.37~1.83;肥胖OR=1.38, 95%CI: 1.13~1.69)。峰值年龄与6岁儿童超重/肥胖之间无统计学关联。结论 峰值时的BMI水平越高,6岁儿童患超重和肥胖症的风险越高,对于学龄前儿童超重/肥胖的预防可从生命早期提供干预措施,以减轻学龄前儿童疾病负担。

Abstract:
Objective To explore the relationship between infant BMI peak, and overweight / obesity in 6-year-old children. Methods Based on the established "Ma’anshan Birth Cohort, (MABC)", follow-up to 6 years of age was continued, and related information was collected by regular physical measurement. BMI growth curves were drawn, and the peak value of BMI along with age were calculated. Logistic regression was used to analyze the relationship between the peak value of BMI in infancy, and its age along with overweight / obesity in 6-year-old children. Results The peak value of BMI in boys was higher than that in girls, and the difference was statistically significant (18.56kg/m2 vs 18.14kg/m2, t=5.155, P<0.01), but there was no significant difference in peak age between sexes (7.10 months vs 7.19 months, t =-1.084, P>0.05). The BMI level was a risk factor for overweight / obesity in 6-year-old children (overweight, OR=1.58, 95%CI: 1.37-1.83, obesity, OR=1.38, 95%CI: 1.13-1.69). There was no statistical correlation between peak age and overweight / obesity in 6-year-old children. Conclusion The greater the peak BMI is, the higher the risk of overweight/ obesity is at age 6. Prevention of overweight / obesity in preschoolers can provide interventions early in life to reduce the burden of disease in preschoolers.
