伊贝沙坦联合辛伐他汀对维持性血液透析患者微炎症状态的影响

目的观察伊贝沙坦联合辛伐他汀对维持性血液透析患者C-反应蛋白(CRP)和肿瘤坏死因子-α(TNF-α)的影响。方法42例维持性血液透析患者随机分为A组(伊贝沙坦治疗)20例,B组(伊贝沙坦+辛伐他汀治疗)22例,治疗前及治疗12周后检测CRP、TNF-α、血浆白蛋白(ALB)、三酰甘油(TG)、总胆固醇(TC)变化。结果A组治疗后ALB较治疗前上升,CRP及TNF-α明显降低,有显著性差异(P均<0.01),TG、TC无明显变化(P均>0.05)。B组治疗前后TG、TC、ALB、CRP及TNF-α均有显著性差异(P均<0.01)。B组治疗后CRP及TNF-α显著低于A组治疗后,ALB较A组治疗后明显升高,有显著性差异(P<0.01)。结论伊贝沙坦可降低尿毒症透析患者血清CRP、TNF-α水平,但伊贝沙坦联用辛伐他汀可更明显改善尿毒症微炎反应,减少尿毒症并发症。

Effect of irbesartan combined with simvastatin on microinflammation in regular hemodialysis patients

Objective It is to observe clinical effect of irbesartan combined with simvastatin on C-reactive protein(CRP) and tumor necrosis factor-α(TNF-α) in regular hemodialysis patients.Methods 42 regular hemodialysis patients were divided into two groups: 20 patients of them were treated with irbesartan(group A)and other 22 patients were given irbesartan combined with simvastatin(group B) for 12 weeks.The CRP,TNF-α,albumin(ALB),triglyceride(TG) and total cholesterol(TC) were measured after 12 weeks.Results The levels of ALB was raised,CRP and TNF-α were decreased,TG and TC had no obviously change in group A.The levels of TC,TG,ALB,CRP and TNF-α had no obviously change in group B.The levels of CRP and TNF-α after treatment in group B were lower,ALB was obviously raised than that in group A. Conclusion Irbesartan can decrease the levels of CRP and TNF-α,but irbesartan combined with simvastatin therapy can more significantly improve microinflammation than irbesartan therapy in regular hemodialysis patients.
