鼻中隔矫正术对改善变应性鼻炎症状的疗效观察

目的观察鼻中隔矫正术治疗变应性鼻炎(AR)的临床疗效。方法以2010年3月至2012年3月在深圳市龙华新区人民医院行鼻中隔矫正术的AR患者30例为研究对象,记录观察术前、术后6个月、12个月、24个月症状与体征、VAS评分、临床疗效及并发症情况。结果患者术后6个月总有效率为96.7%,高于术后12个月(83.3%)及2年总有效率(83.3%),三个时间段疗效比较差异无统计学意义(P>0.05);术后分类症状和总体症状VAS评分,6个月为(3.10±0.41)分、12个月为(3.05±0.33)分、2年为(3.23±0.13)分,均较术前(7.25±1.37)分下降,差异有统计学意义(P<0.05);3例发生黏膜撕裂,2例出现鼻腔黏连,有2例发生鼻中隔血肿,未发现鼻中隔穿孔及鼻腔干燥等情况。结论鼻中隔矫正术治疗合并鼻中隔偏曲的AR患者能够有效的改善的鼻部症状,手术安全可靠、创伤小,术后并发症少,值得临床推广应用。

The Effect of Nasal Septal Reconstruction in Treatment of Allergic Rhinitis

Objective To observe the clinical effect of nasal scptal reconstruction in treatment of aller- gic rhinitis（AR） with nasal septal deviation. Methods A total of 30 cases of AR patients with nasal septal deviation treated with Nasal septal reconstruction in Longhua District People＇s Hospital from Mar. 2010 to Mar. 2012 were selected as object. The patients＇ symptom scores,VAS scores, clinical effects and complications were analyzed before operation and 6 months,12 months and 24 months after operation. Results The overall effective rate in the 6th month after operation was 96.7%, higher than that in the 12th month （83.3%） and the overall effective rate of 2 years （83.3%）, but with no statistically significant difference （P 〉 0.05 ）. The symptom VAS scores of 6 months （3. 10 ± 0.41 ）, 12 months （3.05 ± 0.33 ） and 2 years （3.23 ±0.13） were significantly lower than those of the preoperative visit（7.25 ± 1.37） ,the difference was statistically significant（ P 〈 0.05 ）. Three eases out of all occurred mucosal tearing,two cases occurred nasal cavity adhesion,two cases occurred septal hematoma, while no cases of nasal septum perforation and drying were found. Conclusion Nasal septal reconstruction can improve the nasal symptoms of AR patients with nasal septal deviation. The operation is safe and effective with less complications after operation, which is worthy being recommended in clinical application.
