不确定风湿病患者人类白细胞抗原B27测定

目的调查临床诊断不确定风湿性疾病患者HLA—B27抗原的分布情况，探讨其在风湿性疾病中诊断及鉴别诊断的意义。方法采用标准NIH微量淋巴细胞毒实验方法对747例临床诊断不确定风湿性疾病患者做HLA—B27抗原检测。结果747例中，HLA—B27阳性196例，总阳性率26．24％。HLA—B27阳性者中16岁以下20例(男16例、女4例)，占10．21％；16～40岁162例(男126例，女36例)，占82．65％。大于40岁14例(男9例、女5例)，占7．14％。结论HLA-B27血清学检查对临床诊断不确定风湿性疾病的鉴别诊断具有重要意义．HLA—B27阳性提高了发现AS的可能性。对于诊断不确定的风湿病患者，HLA-B27可作为筛选指标。

Clinical Significace of HLA-B27 Detection in Patients with Un-definition Rheumatic Diseases

Objective To investigate the distrbution of HLA-B_ 27 with un-definition rheumatic diseases,and try to find out the significance of HLA-B_ 27 detection at diagnosis and differential diagnosis in patients with un-definition rheumatic diseases.Methods All the 747 patients with un-definition rheumatic diseases were detected HLA-B_ 27 by standard NIH micro-lymphocyte cytotoxic test.Results Among 747 patients with un-definition rheumatic diseases,196 patients were positive of HLA-B_ 27 ,the positive rate was 26.24 %.In patients with positive of HLA-B_ 27 ,20 cases were below 16 years old(16 male and 4 female),and the percentage was 10.21%;16~40years old were 162 cases (126male and 36 female),and the percentage was 82.65%;14 cases were beyond 40 years old (9male and 5 female),and the percentage was 7.14%.Conclusion HLA-B_ 27 antigen detection is an effective method on AS diagnosis and differential diagnosis in the paitents with un-definition rheumatic diseases.The positive rising of HLA-B_ 27 increased the posibility of AS. HLA-B_ 27 may be a screening marker for AS in patients with un-definition rheumatic diseases.
