超声应变率成像技术评价扩张型心肌病局部心肌的纵向收缩功能

摘    要:目的探讨扩张型心肌病心肌的应变率(SR)变化特点及二尖瓣瓣环的收缩期SR与左室射血分数(LVEF)之间的关系。方法15例扩张型心肌病(DCM)患者与20例正常对照者,将取样容积分别置于心尖位左心长轴切面、两腔切面和四腔切面的左室壁基段、中段和二尖瓣瓣环水平,获取同步的心肌SR曲线,读取收缩期的峰值SR值。用Simpson法测量LVEF。结果DCM组左室各节段心肌的纵向SR明显低于正常组相应节段的SR(P<0.05)。瓣环的SR与LVEF呈正相关(r=0.623,P=0.003)。结论扩张型心肌病患者各部位的心肌纵向SR呈均匀性降低,瓣环的SR可定量评价左室收缩功能。

Abstract:Objective To evaluate the myocardial strain rate (SR) changes on sonography in patients with dilated cardiomyopathy(DCM) and the relationship between the SR of annular and left ventricular ejection fraction (LVEF).Methods Totally 15 patients with DCM and 20 normal subjects were included in this study.The systolic peak SR was restropectively measured at the basal and middle of the left ventricular wall and mitral annular level from apical long-axis,two-chamber and four-chamber views.LVEF was determined by Simpson method. Results The longitudinal SR of left ventricular segments in the DCM group were significantly lower than those in the normal subjects(P<0.05).There was a positive correlation between mitral annular SR and LVEF. Conclusions The myocardial longitudinal SRs of DCM were decreased widely in many segments,and the annular SR can be used to quantitively assess the left ventricular systolic function.
