复杂胫骨平台骨折骨锁定钢板内固定的疗效分析

目的 分析复杂胫骨平台骨折骨锁定钢板内固定的疗效.方法 选择我院2011年5月～2012年10月66例复杂胫骨平台骨折患者,随机分为两组,A组33例患者采用骨锁定钢板内固定,B组33例患者采用常规钢板固定,所有患者均随访3～12个月,对住院时间、骨折愈合时间及完全负重时间等相关指标进行比较,并对膝关节功能恢复情况进行评价,同时观察术后并发症.结果 A组33例患者平均住院时间、平均骨折愈合时间及平均完全负重时间均短于B组,两组比较差异具有统计学意义（P＜0.05）.A组膝关节功能恢复优14例,良11例,优良率为75.76％,B组优8例,良12例,优良率60.61％,A组疗效明显好于B组,差异具有统计学意义（P＜0.05）.A组并发症发生率为6.06％,B组为12.12％,两组均无钢板松动、断钉及骨折不愈合病例,两组并发症比较差异无统计学意义（P＞0.05）.结论 骨锁定钢板具有内固定可靠,骨折愈合快,患者功能恢复好的优点,是复杂胫骨平台骨折的理想方法.

Clinical efficacy of bone locking plate fixation for complex tibial plateau fractures

Objective To investigate clinical effect of of bone locking plate fixation in treatment for complex tibial plateau fractures. Methods A total of 66 patients with complex tibial plateau fraetures in our hospital from May 2011 to October 2012 were divided into the group A with 33 caees, treated by bone locking plate fixation and group B with 33 caces, treated by conventional plate fixation. All patients were followed up for 3-12 months,the hospitalization, fracture healing time , the recovery of knee function, postoperative complications were analyzed. Results The average length of stay, the average healing time and mean time to full weight bearing of group A were shorter than that of group B （P〈0.05）. A group of knee function recovery were excellent in 14 cases, good in 11 cases , good rate of 75.76%, group B were excellent in 8 cases, good in 12 cases, good rate of 60.61%.The good rate of group A was significantly better than that of group B （P〈0.05）.The complication rate of group A was 6.06% and group B with 12.12%, were no plates loose , broken nails and nonunion cases , complications difference between the two groups was not significant （P〉0.05）. Conclusion Locking plate fixation with reliable , fracture healing, functional recovery in patients with good benefits, is an ideal method of complex tibial plateau fractures.
