连续性血液净化治疗重症急性肾功能衰竭

目的观察连续性血液净化(CBP)治疗重症急性肾功能衰竭(ARF)的临床疗效和对预后的影响。方法对2000年以来入我院ICU的13例ARF(伴发多器官功能障碍综合征,MODS)患者进行CBP治疗的临床资料进行回顾性分析。结果9例患者经CBP治疗24h后,生命指征平稳,BUN、Scr下降(P<005),CO2CP正常,75～240h后进入多尿期;4例患者死于严重创伤或多器官功能衰竭(MOF)。治疗过程中无低体温和低血压等并发症。结论对重症ARF患者进行CBP治疗,血液动力学稳定,溶质清除率高,有利于营养支持及清除炎性细胞因子,从而改善重症ARF患者的预后。

Application of continuous blood purification on patients with severe actue renal failure

Objective To investigate the clinical effect and influence on survival of continuous blood purification(CBP) on patients with severe acute renal failure(ARF).Methods The clinical data of 13 patients admitted with severe ARF(with multipie organ dysfunction syndrome,MODS) undergoing CBP in intensive care unit(ICU) from 2000 were reviewed and analyzed.Results 9 patients under-went CBP for 24 hour,with stable life index,decrease of BUN and Scr(P<0.05),normal of CO_2CP,and entered into the period of polyuria after 75 to 240 hours.4 patients died from severe trauma or MODS.There was no complications such as low body temperature and hypotension during the therapy.Conclusion Treatment with CBP on patients with severe ARF might be helpful to stabilize hemodynamics,eliminate the solute,in favor of nutrition abeted,and eliminate inflammation response thereby improving the prognosis of patients' with ARF.
