2011年度四川省血液透析患者死亡情况分析

目的调查四川省血液透析患者死亡情况及原因，以提高血液透析患者的生存率。方法调取国家血液透析登记数据中四川省2011年透析登记患者7067例，男性4219例，女性2848例，平均年龄52±17岁。分析死亡患者人数、肾脏病病因、死亡原因。结果透析登记结果显示死亡人数148人，中位透析龄11月，平均年龄60±16岁；男性占60．1％（89人），女性占39．9％（59人）。死亡原因分别为：心脑血管疾病占55．4％，感染性疾病10．8％，消化道出血等出血性疾病7．4％，肿瘤晚期4．1％，自杀1．4％，其他原因占20．9％。女性患者自杀占3．4％，男性则无一例自杀。其中69％患者合并有1个及以上慢性肾脏病并发症或合并症。结论四川地区血液透析死亡患者年龄相对较高、有较多合并症，主要死亡原因是心脑血管疾病、感染及出血，其中女性患者有较高的自杀死亡率。

Study on mortality of hemodialysis patients of Sichuan province in 2011

Objectives To study the mortality and causes of death of hemodialysis patients in Sichuan province for improving patient survival.Methods Information data of Sichuan hemodialysis patients were exported from Chinese national renal data system,2011.7067 patients were enrolled including 4129 male and 2848 female patients,the average age were 52±17 years.Mortality rate,causes of mortality and original disease of chronic kidney disease were analyzed.Results 148 patients died in 2011 with median hemodialysis duration of 11 months,an average age 60±16 years.There were 89(60.1%) male patients and 59(39.9%) female patients died.The main causes of mortality were cardiovascular disease(55.4%),infectious diseases(10.8%),hemorrhage disease including gastric bleed(7.4%),malignancies(4.1%),suicide(1.4%) and other uncertain causes(20.9%)。There was a higher suicide rate(3.4%) in female patients than male patients(none).69% patients had more than 1 complication or comorbidity.Conclusions Hemodialysis patients in Sichuan province with older age and more complications or comorbidities are at higher risk for death.Cardiovascular disease,infectious disease and hemorrhage disease are the main causes of mortality.Special attention should be paid to reduce suicide rate for female hemodialysis patients.
