切开复位内固定治疗Lisfranc关节损伤

目的 总结切开复位内固定治疗Lisfranc关节损伤的临床经验,以提高对该处损伤的治疗水平.方法 自2002年6月至2004年11月期间,我院共有51例计57侧Lisfranc关节损伤的患者接受了切开复位内固定术.患者平均年龄29.6岁,损伤按Quenu-Kuss分型,A型损伤(同向移位)11侧,B型损伤(部分移位)36侧,C型损伤(分裂移位)10侧.手术采用1～2个足背纵形直切口,内植物材料包括螺丝钉、克氏针及AO小钢板.结果 术后平均随访23.8个月(4～32个月),根据Maryland足部功能评分(MFS)评估疗效,优占14侧(24.6%),良28侧(49.1%),可10侧(17.5%),劣5侧(8.8%),总体优良率为74%.结论 使用切开复位内固定治疗Lisfranc关节损伤,可以取得良好的效果.

ORIF for the treatment of lisfranc's joint injuries

Objective To summarize the clinical experience of open reduction and internal fixation (ORIF) for the treatment of Lisfranc's joint injuries, raise the treatment level of the local site. Methods Between June 2002 and November 2004, a total of 51 patients with 57 injuries were treated by ORIF. On dorsum of foot, 1 or 2 longitudinal straight incisions were used during surgery. Implants includes screws, Kirschner wires and AO plates. The patients' average age was 29. 6. Injuries were classified by Quenu-Kuss classification, included 11 A-type (homolateral), 36 B-type (isolated), 10 C-type (divergent). Results When evaluated by Maryland Foot Score (MFS). a mean follow-up of 23. 8 months (4 to 32 months) revealed that excellent results occurred in 14 injuries (24.6%), good in 28 (49. 1%), fair in 10 (17. 5%) and poor in 5 (8.8%). The total excellent and good results were 74%. Conclusions ORIF for the treatment of Lisfranc's joint injuries should have satisfying results.
