老年胆囊疾病患者腹腔镜胆囊切除与开腹手术治疗的疗效分析

目的比较老年胆囊疾病患者腹腔镜胆囊切除术（LC）与开腹胆囊切除术（OC）的手术效果,分析LC的临床优点和探讨老年胆囊疾病适宜的手术方法。方法 92例老年胆囊疾病患者据手术方式分为LC组（n=46）和OC组（n=46）。比较两组患者手术时间、手术出血量、术后通气时间、下床时间、术后镇痛泵使用情况、伤口感染、住院天数以及手术成功率。结果两组手术成功率相似,无统计学意义（P〉0.05）。但其他观察指标LC组明显优于OC组（P〈0.01）。结论 LC具有创伤小、手术时间短、出血量少、痛苦轻、术后恢复快、伤口感染率低等优势。但老年患者的手术治疗应遵循个体化原则,是行OC还是LC要视患者的总体情况而定。
